Convergence insufficiency: the reason behind eyes that tire while reading

In short

In convergence insufficiency the eyes struggle to sustain focus up close, producing fatigue, headaches and avoidance while reading. It differs from lazy eye and is treated differently, but only an exam can tell them apart.

Some children grow restless shortly after starting to read: they rub their eyes, push the book away, skip lines, and eventually give up. The conclusion usually reached is an attention problem, when a visual strain may sit behind it.

The best-known name for this picture is convergence insufficiency, and it is a different condition from lazy eye.

What does looking up close require?

When you look at a book, both eyes turn inward to meet at the same point; that is convergence. At the same time the lens does the focusing.

Both adjustments have to be sustained for as long as the reading lasts. If the system is under strain, the first few minutes go fine and performance then drops. That is why complaints appear in the middle of reading rather than at the start.

What does it look like?

The typical complaints are confined to near work: tired or burning eyes, headaches, losing the line, letters starting to drift or double, wanting to cover one eye, avoiding reading.

Distance vision is usually fine. So the sentence my child sees the board clearly does not show that near work is trouble-free.

The behavioral picture misleads too: a child who gets bored quickly, dodges homework and keeps shifting position may in fact be struggling visually.

How it differs from lazy eye

In lazy eye one eye's visual development has fallen behind, and the problem usually comes without complaints. In convergence insufficiency the acuity of both eyes can be normal; the difficulty lies in sustaining teamwork up close, and it does produce clear complaints.

The two do not exclude each other and can occur together. The only way to separate them is an exam, where the near point of convergence is measured and the eyes' teamwork assessed.

How is it handled?

The first step, as always, is correcting the refractive error properly. After that, and according to the doctor's plan, work aimed at the eyes' teamwork may come into play.

Small adjustments at school help too: good lighting, short breaks during reading, splitting long texts into chunks. These do not replace treatment, they lighten the load.

If your child has complaints like these, taking them to an exam rather than filing them under reluctance is the right move.
