What strabismus surgery does, and what it does not

In short
Strabismus surgery is an alignment procedure: the muscle positions are changed so the visual axes run parallel. It does not treat amblyopia and does not raise acuity in the weaker eye. That is why the refractive error and the amblyopia are usually addressed first and alignment discussed afterwards. Learning to use the two eyes together remains a separate process after the operation.
Strabismus and lazy eye are mentioned in the same breath so often that the surgery gets misunderstood just as often. The question parents ask most is whether an operation will clear up the lazy eye too.
The short answer is no. Surgery changes where the eyes point, not how sharply they see. This article explains the difference and why the order of treatment matters. The decision to operate always rests on your eye doctor's assessment.
What the operation actually corrects
Six muscles move each eye. In strabismus their balance is off, and one eye does not turn towards the point the other is fixing on. The surgeon restores that balance by shifting where a muscle attaches or by shortening it.
The result is a correction of alignment. Once the eyes are parallel, appearance improves, double vision resolves in some children, and the groundwork is laid for the brain to merge the two images.
Groundwork is the key word. Alignment is a precondition for fusion, not fusion itself.
Why it does not treat amblyopia
Amblyopia is a problem of the brain's visual pathways, not of the muscles. The weaker eye's image went underused during the developmental years, so its signal stayed in the background. Moving a muscle does not strengthen that signal.
Acuity therefore does not rise on its own after surgery. Whatever was needed to raise it is still needed: glasses, penalisation or dichoptic training.
The reverse holds as well. Treating the amblyopia does not necessarily straighten the eyes. The two conditions often live in the same child, but they are separate problems and each gets its own treatment.
Why the order matters
Doctors usually deal with the refractive error and the amblyopia first, for two reasons. The first is accommodative esotropia, where correcting the hyperopia removes much of the turn and sometimes all of it, which can make surgery unnecessary.
The second is that as the weaker eye's vision improves, the chances of the brain actually using both eyes go up. Alignment is more likely to hold when the two eyes see at similar levels.
This is a general frame rather than a rule. Large-angle or early-onset deviations may bring surgery forward. The decision follows the type of deviation and the child's age.
What to expect in practice
In children the operation is done under general anaesthesia and is usually a day case. Redness and a gritty feeling can last a few weeks.
Temporary double vision in the first days is common while the brain adjusts to the new alignment.
A single operation does not always achieve the full result. Because muscles respond differently from one person to the next, some deviation can remain and a second procedure may be discussed. That is variability inherent to the work, not a failure.
Is the work finished after surgery?
Alignment fixes the hardware, not the software. Once the eyes are parallel, the brain still has to learn to merge the two images and rebuild depth, and in an eye that has been suppressed for a long time that does not happen by itself.
This is exactly where dichoptic training earns its place: it works both eyes at once and aims at the teamwork itself. It does not replace the operation; it makes use of the opening the operation created. Plan the sequence with your doctor.
